Align SERVICES
We provide case management services which includes assistance with the Medicaid Application, finding and establishing services in your community, and monitoring services and overall well being with a monthly or quarterly home visit.

Part of Wyoming Medicaid, the Home and Community Based Services (HCBS) Section oversees waiver programs for individuals with developmental disabilities, acquired brain injuries, physical disabilities, and more.
